LIABILITY WAIVER AND NOTICES: I understand that participation in this event or activity could include actions or tasks which mights be harmful to the participant named above. By signing below, I assume any risk of harm or injury which might occur to the participant due to his/her participant in the event or activity listed above. I release HEXIS and SWP SOLUTION, INC. from all liability, costs, and damages which may arise from participation in the above-named event or activity. I acknowledge, I am responsible for all travel accommodations to attend this event or activity. I acknowledge if HEXIS and/or SWP SOLUTION, INC. cancels this event or activity, my money (paid for the training class or certification test) will be refunded in the original form payment was received in. HEXIS and SWP SOLUTION, INC. are not responsible for lost funds or fees for canceled/rescheduled flights, hotels or accommodations. I, the participant, acknowledge that refunds will not be given if I, the participant, need to cancel or reschedule attendance to the training class or certification test. If I, the participant, need to cancel my attendance to the event or activity my reservation will be moved to the next available training class or certification test. I, the participant, acknowledge and understand that if a refund is issued due to duplicate payments, customer error, or any situation necessitating a refund, a 3% + $0.30 refund processing fee will be deducted from the initial payment. I acknowledge that I will not share or promote the exclusive student discounted price list for HEXIS products with any third party or on social media. I understand that any violation of this agreement will result in the forfeiture of my access to the exclusive pricing. I acknowledge I am attending this class at my own free will during a global health pandemic. HEXIS and SWP SOLUTION, INC. are not liable for any sickness that may be transmitted to the above participant during travel, training classes or certification test including COVID-19 or other communicable disease or reasonable threat thereof, influenza or reasonable threat thereof, disease outbreak with warnings by the World Health Organization, US State Department, Center for Disease Control or Department of Homeland Security. COVID-19 Protocol: HEXIS and SWP SOLUTION, INC. may perform temperature checks upon arrival of our facility. If you have a temperature of 100 degrees Fahrenheit or higher, SWP SOLUTION INC. and HEXIS Americas reserves the right to decline entry into our facility. Therefore, you will be unable to participate in training or certifications. HEXIS and SWP SOLUTION, INC. are not liable for lost funds or travel costs due to being turned away due to fever. Individuals who have had symptoms in the last fourteen (14) days will not be able to attend training classes or certification tests. Lastly, I acknowledge, my spot for the referenced training class or certification test is not secured and confirmed until 100% payment has been made and all forms have been submitted. Photo & Video Release Disclaimer: I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area. By submitting this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.
